SOAP drafts
Synthesizes complaints, history, examination, assessment and plan from spoken dialogue. On mobile and web.
Tabib.today is an AI assistant for clinicians in Uzbekistan. It records the consultation and prepares a SOAP draft. You review, edit and confirm it yourself.
Many doctors write documentation after hours: retyping into paper charts or DMED. Over 40% of consultation time is spent facing the screen.
Retyping history, examinations, and plans after consultations. 15-20 minutes of repetitive paperwork per patient.
Looking down at screens while the patient speaks reduces clinical eye contact and rapport.
What did the patient report last month? Lost paper charts and scattered notes waste critical visit minutes.
One tap on your smartphone records the visit. Pause and resume freely. Offline-safe if Wi-Fi drops.
The secure server drafts structured SOAP notes (Subjective, Objective, Assessment, Plan) from the conversation.
Every note point is linked to exact audio dialogue. Click “Listen” to verify any statement instantly.
You confirm the final record. Confirmed records remain immutable, with subsequent updates logged as addenda.

Cardiology, endocrinology, general practice and universal templates. Clinics can customize their own forms.
Pre-visit executive brief of the patient’s last 3 finalized visits, with citations linked to each note.
Recognizes mixed languages, local clinical vocabulary, and medical jargon. You select document language.
Tabib.today does not replace the physician: it accurately documents only what you clinically conclude.
A diagnosis is recorded only if spoken aloud by the physician. Otherwise, the field remains blank.
Every synthesized phrase links to an exact audio fragment. Verify any nuance with one click.
AI never finalizes or archives notes automatically. The clinician reviews, corrects, and confirms.
Tabib.today and Tablet.today are two sides of one connected platform. Tablet.today reminds patients to take medications; Tabib.today empowers doctors during visits. Connected securely via QR pairing and explicit patient consent.
Sample data
The patient scans the physician’s QR code in Tablet.today and authorizes clinical link access.
The doctor views the profile: active medications and real-world dose adherence percentage (PMR).
The patient consents to visit recording. The physician records and reviews the generated SOAP draft.
A confirmed plain-language summary is sent to the patient’s app. Pre-visit briefing refreshes the next visit.
The patient can revoke doctor access at any time with one tap. Raw medical records (EMR) are never shown to patients - they only receive physician-approved plain-language summaries.
Patient consent is re-verified at every visit. Unenrolled patients provide recorded verbal consent.
All audio streams, transcripts, and clinical drafts are secured with end-to-end cryptographic encryption.
Unconfirmed consultation audio, raw transcripts, and draft files are permanently deleted after 72 hours.
Every read, edit, and access event is recorded in a tamper-resistant security audit trail.
Structured for blood pressure dynamics, ECG findings, heart rate curves, and medication response.
Optimized for glucose profiles, HbA1c metrics, thyroid markers, and dietary adjustments.
Fast-paced templates for broad clinical complaints, acute illness, and routine preventive exams.
Universal medical SOAP template or custom departmental forms configured for your clinic.
No. Tabib.today acts strictly as an ambient clinical assistant. A diagnosis is written only if the physician speaks it aloud; otherwise the diagnosis field remains empty. ICD-10 suggestions require manual physician confirmation.
Patients grant consent through the Tablet.today app via the physician’s QR code, reconfirmed at each visit. For patients without smartphones, verbal consent is recorded at the start of the audio session.
